How to search and login into your provider’s Mychart portal?
If you are a registered member then you can easily access your patient portal with the help of the below-mentioned steps.
Step 1: Open the official website.
Step 2: Click the Access MyChart link available on the homepage and at the top right corner of the website. You will see an interface as shown below.
Step 3: You can now use two methods to connect to your patient portal.
i-Through the locations mentioned below the search form:
Click any location or country name given below this page. On the next page, you will see a list of all healthcare providers along with the Login and Sign-Up link.
To log in, click the Login link and you will be redirected to the login page of your provider. For example, if you click the CVS Health MyChart Login link, you will be redirected to the CVS Health login page (as shown below).
On the login page, Enter your username, and password and click the Sign In button to access your patient account.
ii-Type and search hospital, or location:
You can also directly type your hospital or clinic name in the search form and the Sign In link will appear below the search form if your hospital is using the Mychart software.
How to reset your password?
In case you forgot your password then you can reset it through the Mychart login page of your provider. Let’s take the example of CVS health.
- Click the Forgot Password link located just below the login form.
- A password reset form will appear on the next screen (as shown below).
- In this password reset form, enter your Mychart username, ZIP code, and date of birth, and press the Next button.
- You will receive an email including a password reset link to reset your Mychart password.
- Just click the password reset link and you will be able to create a new password on the next screen.
How to recover the Mychart username?
Please follow the below steps to easily recover your username in case you don’t remember it.
Step 1: Open the login page of your healthcare provider through the steps mentioned above.
Step 2: On the login page, click the Forgot username link, On the next page, a username recovery form will appear (as shown below).
Step 3: In the username recovery form, enter your First name, last name, ZIP code, and date of birth and press the Submit button. You will soon get an email including your username.
Note: In case you don’t remember any information such as your username, password, and email ID then you will have to contact the Mychart customer support team of your healthcare provider. Some providers share the Customer support contact information on the username recovery page and some do not.
How to create a new MyChart account?
You will have to register a Mychart account if you want to access your clinical information, health-related data, and other useful services online. You will have to create a separate Mychart account for a different provider. The registration process is almost the same for all providers except for some minor changes.\

Step 1: Search for your healthcare provider on the search page www.mychart.com.
Step 2: Once the information of your provider appears below the search form, click the Sign-Up button. If the Sign-Up link is to available or not working then first open the Login page and click the Sign-Up or create an account link there.
Step 3: A new online registration form will appear on the next screen (as shown below).
Step 4: Fill in all required information in this form and create a new username and password to access your account in the future. Finally, submit this form for verification purposes.
How to activate a new account?
Some providers use the account activation step to authenticate your account. You can easily activate your Mychart account through the activation code which you will receive after the registration. Please follow the below steps.
Step 1: Open the login page of your health provider.
Step 2: Click the account activation link located on the login form. A new account activation form will appear on the next screen. (as shown below).
Step 3: Enter the activation code (which you receive in your email after the registration), ZIP code, and date of birth. Click the Next button and your account will be activated instantly.
