Ascension Mychart (previously known as Mywheaton Chart). After the acquisition of Wheaton Franciscan Healthcare by Ascension Wisconsin, All patients are using the new patient portal Ascension Mychart. Previously they were using MyWheaton Chart.

Apart from the Mychart, Ascension is using different patient portals as per the different states. If you were previously using MyWheaton Chart then you will use the new Ascension Mychart web portal. For more information about the different patient portals

How to access Mywheaton Chart (Now Ascension Mychart)?

Ascension has a national presence and provides its healthcare services in different states. You must have the correct web URL of your patient portal. To help you out, we are sharing details on how to find your patient portal.

  • Please visit the Ascension website
  • Click the Patient Portal link in the top bar.
  • Alternatively, Visit the patient portal web page directly at if the link in the top bar is not working,
  • On the patient portal web page, select your state (Wisconsin).
  • You will find different patient-related links including the link for your patient portal.

How to log in?

Enter your Mychart username and password and press the Sign In button to access your patient portal. Since it’s a Mychart portal which is a popular Electronic Health Record (EHR) system in the United States so all of your patient data will be safe.

If you have not registered an account then first sign up for a new patient account and create your username and password.

How to activate Ascension Mychart (formerly Mywheaton Chart)?

Patients can use the online sign-up method with the help of an activation code to instantly get access to the patient portal. You must be 18 years or older to sign up for a new account.

Sign up with an activation code

When a patient visits any Ascension healthcare facility for the first time then Ascension shares an activation code with the patient. Patients can use this activation code to instantly sign up and activate their patient accounts. The activation code is generally given through an enrollment letter, after-visit summary, or email.

If you have also visited the first time and got your activation code then simply visit the Mychart activation web page and enter your activation code, date of birth, and social security number, and complete the activation process. You can create new login credentials during the activation.

How to recover your Mywheaton Chart (Ascension Mychart)?

Account recovery options are available for patients so that they can recover their login credentials in case of incorrect usernames and password errors.

Patients can find the password reset and username recovery options on the Mywheaton Chart login page. Please follow the below simple steps to recover your account.

  1. Visit the Mywheaton Chart login page.
  2. Click the Forgot Username or Forgot Password option available on the login form.
  3. You can recover both your username and password by sharing your account-related information such as your name, social security number, and date of birth.

After downloading the mobile app, patients can log in to their accounts by using the same username and password that they are using on the web portal. After login, patients can use Face ID or fingerprint scan to directly log in to their account in place of entering the login credentials.
